We encourage all funding assistance, such as NDIS support, GP medicare referrals and private health insurance.
Private Health Insurance
You can claim your rebate through your private health insurer.
Please note that rebates vary according to private health funds and your level of cover. You will need to contact your health fund directly to determine your rebate.
NDIS
Active Strides can assist NDIS participants who are plan managed and/or self-managed.

Medicare
Chronic disease management plan (CDMP)/Team Care Arrangement (TCA) – code 10960
- Medicare rebates are available for patients who have obtained a CDMP & TCA through their General Practitioner
- Patients can access rebates for up to 5 visits per calendar year
